You will need:
DVD Decrypter
ISO Buster
Sony CD/DVD ROM Generator
Apache2

1/ Make The Backup
Open DVD Decrypter and Left Click Mode > ISO > Read if its not already done,
NOTE: You can skip this step if you just put your game into the driver and open ISOBuster.

Insert your game into your DVD Driver. Clic the destination folder to put the ISO for easy acces like "FIFA06" if its Fifa 06 and name the iso fifa06, press ok. Now press the DVD Decrypt button which will take up to several minutes to burn the ps2 ISO to your hard drive.

2/ Close DVD Decrypter and open ISOBuster

File > Open Image File and look for the ISO you just decrypted, it will show you the whole content of the ISO.

Select all files and folders like the picture then Right clic > Extract Objects.
Extract them to a folder you would remember. You better forget space and caracter in the path (C:\FIFA06\ seem the best choice).
Keep ISOBuster open for later use.

3/ Open CD/DVD ROM Generator.

Make a New Project, select DVD-ROM Master Disk. Now open My Computer and go to your hard drive and look for the files you just extracted with ISOBuster. Place ISObuster in a corner of the screen, CD/DVD Gen in another and the folder with your files visible (helpful to slide-in).

You will need to place every files you see in ISOBuster into CD/DVD Gen with the EXACT SAME ORDER including folders and subfolders. Do it one at the time.

After all files is placed in correct order, select the Volume Tab.

You will be taken to a new window, these next steps will be divided into three sections: (see the pic)

1° Disk Name: it's like the ID of the game, put the same numbers and letters that you saw in ISOBuster

 NOTE: American games (NTSC) are like SLUS_XXX.XX and SLES-XXX.XX for European (PAL) i don't know for japan.
So i got SLUS_214.08, the disk name will be "SLUS-21408".

2° License Area: it depends where you are, if you're in Europe put Europe, America put America and so on ...

3° Volume: Name for you ISO (you can put anything).
You should have something like this:

Now select "File > Export iml file"

And save it in a folder. What about "C:\FIFA06\ISO\" (the ISO will be create in the same folder).
NOTE: You can clic the RECORD tab:
Once saved, you obtain 2 files: file.iml and file.ims.

4° Open Apache2

Select "File > Import IML File" and clic the iml file you just created.

You'll have this:

Again press "File > Build ISO" and select the path where you want to put the ISO file. (remember: C:\FIFA06\ISO\)
You will see the pourcentage of you new ISO at the bottom of the window. (I'll call it the "ISO bar")

5° CONGRATULATIONS !! YOU JUST REBUILT YOUR GAME !!

6° Open DVD Decrypter
Clic "Mode > ISO > Write" and select you newly created ISO.

Put a DVD-R in your DVD Burner, you will see the picture changing
from this: to this:

Don't forget to burn at the LOWEST SPEED !!  I know it still work if you burn faster
but not all games support fast burn speed so now you're aware.
PS: I use media maxell & TDK DVD-R.

Now clic the "HD to DVD" picture and you are good to go !

Once it's finish, well ... load your Boot Method and then your new backup!
